This is what I can come up with on the intro for Beer with Jesus.  There is more picking involved but this is the basic.
Use your pinky for the hammer ons.   The last part is a D chord but I am still not 100% if that is the right strings for that.
Any suggestions would be appreciated!

*For the video this only goes through once but for some recordings the intro is played twice.  When you are doing it twice do not add the D chord at the end-stay on C and repeat adding the D the second time.
