Alright, so here it goes. I'm not sure I know the names of all the chords, but 
I'll try to put up a figure of the fingering of each chord. The song is in 3/4 
time. I'll try and place the chords above the lyrics. I haven't done one in a 
long time, so go easy on me, plus I'm doing this all from memory. So here

That jazz chord is actually just a C#7 chord.
